Lymphorrhea is the leakage of lymph-rich fluid through the skin. It most often appears in a markedly swollen lower extremity as clear or straw-colored droplets, persistent weeping, or enough fluid to saturate clothing and dressings. In UK and international literature, the term is often spelled lymphorrhoea; clinicians may also hear patients describe the problem as “leaking legs,” “weeping legs,” or “wet legs.”

The most important clinical point is this: lymphorrhea is not simply a dressing problem. It is a visible sign that the skin barrier, tissue-fluid balance, and underlying edema require assessment. Effective care usually combines medical screening, protection of the skin and wound, management of excess moisture, appropriately selected compression when safe, movement or positioning strategies, and treatment of the condition driving the edema.

A dressing may contain the fluid. It does not, by itself, reduce the tissue pressure producing the leak.

For clinicians, the immediate priorities are to:

  1. Screen for infection, vascular compromise, thromboembolism, cardiopulmonary instability, and other urgent concerns.
  2. Determine why the edema or leakage changed.
  3. Assess the wound, surrounding skin, limb, circulation, sensation, and functional context.
  4. Control moisture without traumatizing fragile tissue.
  5. Select and monitor compression according to perfusion, diagnosis, limb shape, drainage, and patient tolerance.
  6. Coordinate care and plan for edema control after the skin closes.

The 2026 clinical review of lymphorrhea emphasizes the same integrated approach: holistic assessment, skin protection, positioning and activity, timely therapeutic compression, and dressings capable of managing the fluid burden.

What Is Lymphorrhea, and Why Does Fluid Leak Through the Skin?

Lymphorrhea occurs when fluid escapes through an impaired skin barrier in an area of edema. The NHS Highland wet-legs guideline describes it as clear lymphatic fluid leaking through the skin, often in beads or a trickle. The amount may range from intermittent dampness to heavy, continuous drainage.

In a chronically swollen limb, several changes can make leakage more likely:

  • Interstitial fluid volume and pressure increase.
  • The skin becomes stretched, fragile, inflamed, or macerated.
  • Hyperkeratosis, papillomatosis, fissures, or small erosions may compromise the barrier.
  • A minor scratch, blister, or pinhole opening can become an exit point for fluid.
  • Reduced mobility and prolonged leg dependency can limit the muscle-pump activity that supports venous and lymphatic return.
  • Venous hypertension may continue to drive filtration into tissues faster than an impaired lymphatic system can clear it.

Lymphorrhea can occur in primary or secondary lymphedema, but chronic venous disease and lymphatic overload frequently coexist. Clinicians who see persistent lower-extremity edema may find it helpful to review phlebolymphedema and the relationship between venous and lymphatic dysfunction.

Common Contributors to New or Worsening Leakage

The leak may follow a gradual progression of uncontrolled edema, but it can also appear after an abrupt change. Potential contributors include:

  • Increased time sitting with the legs dependent
  • Reduced walking or a sudden decline in mobility
  • An interruption in the patient’s usual compression routine
  • A poorly fitting, worn, or insufficient compression system
  • An acute illness or hospitalization
  • Cellulitis or another inflammatory process
  • Progression of chronic venous insufficiency
  • New or worsening cardiac, renal, hepatic, or endocrine disease
  • Medication changes that contribute to peripheral edema
  • Poor nutritional status or hypoalbuminemia
  • Trauma, a blister, dermatitis, fungal infection, or another break in the skin
  • Obesity-related lymphatic overload
  • Malignancy, recurrent disease, or another source of lymphatic or venous obstruction

These possibilities are not interchangeable. A patient with stable bilateral chronic edema who stopped wearing compression after losing caregiver support needs a different response from a patient with sudden unilateral swelling and pain, or from a patient whose leakage appeared alongside new orthopnea and rapidly increasing bilateral edema.

Why “Leaking Legs” Should Trigger Assessment, Not Just a Dressing Change

Persistent moisture can macerate the surrounding skin, enlarge superficial openings, and weaken an already vulnerable barrier. The leaking site can also provide an entry point for bacteria. Chronic edema and impaired lymphatic drainage are recognized risk factors for cellulitis, according to the Centers for Disease Control and Prevention.

The effects are not only physical. Lymphorrhea may soak clothing or bedding, create odor concerns, restrict activity, disturb sleep, and make patients reluctant to leave home. Frequent dressing changes can add cost and caregiver burden. A bulky or unreliable dressing plan may also make footwear and mobility more difficult.

What Should Clinicians Assess First in a Patient With Lymphorrhea?

Begin with the entire patient, not the drainage alone. Lymphorrhea may be the most visible finding, but the first decision is whether routine wound and lymphedema care is appropriate today.

1. Screen for Findings that Require Urgent Medical Evaluation

The following features should change the plan from routine edema management to prompt escalation according to the clinician’s setting and local protocol:

2. Establish the Timeline and What Changed

A focused history should clarify:

  • When did the leaking begin?
  • Was onset gradual or sudden?
  • Is drainage increasing, decreasing, or fluctuating?
  • Is swelling new, or is this a change from the patient’s usual baseline?
  • Is one limb involved or both?
  • Did the patient recently become less mobile or begin sleeping in a chair?
  • Was compression stopped, changed, or no longer tolerated?
  • Was there a recent hospitalization, operation, fall, wound, or infection?
  • Are there new pain, warmth, redness, fever, chills, or flu-like symptoms?
  • Are there new breathing symptoms, chest symptoms, rapid weight changes, or reduced exercise tolerance?
  • Has the medication list changed?
  • Is there a history of venous disease, DVT, peripheral artery disease, diabetes, neuropathy, heart failure, kidney or liver disease, cancer, lymph-node surgery, or radiation?
  • Has the patient’s ability to perform skin care, dressings, compression, or mobility tasks changed?

Social and practical information matters. A plan that requires frequent skilled reapplication may fail if the patient has no caregiver, limited transportation, impaired hand function, cognitive barriers, or inadequate access to supplies. Those are treatment variables, not afterthoughts.

3. Examine Both Lower Extremities and the Whole Edema Pattern

Inspecting only the wet area risks missing the larger diagnosis. Compare both legs and assess:

  • Distribution of swelling, including the feet, toes, ankles, calves, knees, thighs, and genital or abdominal region when relevant
  • Pitting, tissue firmness, fibrosis, skin-fold depth, and contour distortion
  • Limb circumference or volume using a reproducible method when feasible
  • Varicosities, hemosiderin staining, lipodermatosclerosis, or other signs of chronic venous disease
  • Foot and toe involvement and whether edema extends beyond the area covered by current compression
  • Joint motion, gait, calf-muscle function, balance, and usual activity level
  • Pain location, quality, severity, and whether it is consistent with the visible findings

Redness is not automatically cellulitis. Stasis dermatitis, acute lipodermatosclerosis, contact dermatitis, fungal infection, inflammatory skin disease, and heat exposure may mimic infection. Conversely, the absence of fever does not exclude cellulitis. The clinician should identify a change from baseline, assess the full symptom pattern, and refer when the diagnosis is uncertain. 4. Characterize the Leakage, Wound, and Surrounding Skin

Document the location and number of leaking areas, then assess:

  • Color, consistency, odor, and estimated volume of fluid
  • Frequency of strike-through or dressing saturation
  • Whether leakage comes through apparently intact but porous skin, a fissure, blister, erosion, ulcer, surgical site, or traumatic wound
  • Wound dimensions, depth, tissue type, undermining, and wound-edge condition when an open wound is present
  • Maceration, denudement, dermatitis, hyperkeratosis, papillomatosis, or fungal changes
  • Interdigital spaces, heel, plantar foot, and skin folds
  • Evidence of pressure injury from footwear, bandages, tubing, or garment edges
  • Tenderness, fluctuance, purulence, malodor, or rapidly worsening tissue

Clear or straw-colored fluid supports the clinical impression of lymphorrhea, but color alone does not establish the cause. Wound exudate, serous drainage, infection, and other sources of fluid may coexist. The assessment should explain both the opening and the edema driving fluid toward it.

5. Assess Sensation and Arterial Perfusion Before Choosing Compression

Compression is often central to controlling lymphorrhea, but its safety and dosage depend on the circulation beneath it.

The examination should include relevant findings such as:

  • Foot temperature and color
  • Capillary refill interpreted in context
  • Rest pain, nocturnal foot pain, or pain worsened by elevation
  • Claudication history
  • Nonhealing toe, foot, or ankle wounds
  • Tissue loss or gangrenous change
  • Prior revascularization or known peripheral artery disease
  • Diabetes, renal disease, neuropathy, or other factors that may alter sensation or make arteries noncompressible

When findings or history suggest peripheral artery disease, objective testing may include an ankle-brachial index, Doppler waveforms, toe-brachial index, toe pressure, or other perfusion measures. The 2024 multisociety guideline for lower-extremity peripheral artery disease recommends resting ABI testing when the history or examination suggests PAD and toe-pressure/TBI assessment when arteries are noncompressible.

An ABI should not be treated as a stand-alone permission slip. Calcification, edema, technique, wound location, symptoms, and waveform quality can affect interpretation. Abnormal, noncompressible, inconsistent, or clinically discordant results warrant further evaluation and, when appropriate, vascular consultation.

An international consensus statement on compression risks and contraindications recommends evaluating arterial circulation before compression and paying particular attention to patients with impaired perfusion. The safest takeaway is not a universal pressure or cutoff for every patient. It is that compression must be selected by an appropriately trained clinician using the patient’s vascular findings, systemic status, diagnosis, local protocol, and response to treatment.

What Conditions May Be Driving the Edema?

“Wet legs” describes an appearance; it does not identify one cause. Chronic lower-extremity edema is commonly multifactorial.

Venous and Lymphatic Dysfunction

Venous hypertension increases filtration into the tissues. Over time, the lymphatic system may be unable to keep pace, contributing to phlebolymphedema, inflammatory skin changes, fibrosis, and leakage. A patient can therefore need both venous and lymphatic reasoning rather than being placed into only one category.

Immobility and Dependency

Calf-muscle contraction and ankle movement support venous and lymphatic return. A sudden reduction in walking, prolonged sitting with the feet down, or sleeping upright may worsen dependent edema. The change may be especially important when paired with interruption of compression.

Systemic Disease

Cardiac, renal, hepatic, endocrine, and nutritional conditions may contribute to bilateral or generalized edema. Treating only the legs without recognizing a change in systemic status may be ineffective or unsafe. Laboratory testing, medication review, or medical management may be needed.

Medication Effects

Several medication classes can contribute to edema. Clinicians should document recent additions or dosage changes and communicate concerns to the prescribing provider. Patients should not be told to stop prescribed medication on their own.

Infection, Inflammation, and Skin Disease

Cellulitis may sharply increase local swelling. Dermatitis, fungal infection, and scratching can damage the barrier and create leakage sites. Diagnosis matters because antibiotics, antifungals, topical anti-inflammatory treatment, wound care, or other medical management have different indications.

Venous Obstruction, DVT, or Malignancy

New unilateral swelling, abrupt progression, unexplained pelvic or abdominal symptoms, or a substantial change in a previously stable patient may justify medical investigation for obstruction. A clinician does not need to diagnose the cause to recognize that routine decongestion should pause until it is evaluated.

How Does Compression Help Lymphorrhea?

When it is safe and properly selected, compression helps address the edema that sustains leakage. It can:

  • Increase external tissue pressure and limit further fluid accumulation
  • Support venous and lymphatic return
  • Improve the effectiveness of the calf-muscle and joint pumps during movement
  • Reduce limb volume and tension on fragile skin
  • Help create conditions in which superficial openings can close
  • Maintain improvement after active leakage resolves

The International Society of Lymphology’s 2023 consensus document includes compression as a core element of conservative lymphedema management. The All-Ireland Lymphoedema Guidelines specifically pair wet-leg care with skin care, absorbent dressing, vascular assessment, movement, and appropriately selected inelastic compression.

Dressings Manage Fluid; Compression Helps Manage the Pressure Behind It

An absorbent dressing has an important job: receive fluid, retain it under compression, protect clothing, and reduce prolonged moisture exposure. A non adherent wound-contact layer may help avoid stripping fragile tissue during changes. Periwound protection may reduce maceration or irritant dermatitis.

But a dressing that repeatedly becomes saturated while edema remains uncontrolled is managing an output rather than the process producing it.

The dressing plan should be selected according to:

  • Drainage volume and viscosity
  • Wound dimensions and location
  • Skin fragility and sensitivity
  • Capacity to retain fluid under compression
  • Bulk beneath the compression system
  • Change frequency and access to care
  • Patient comfort, mobility, footwear, and cost

There is no single dressing that is best for every lymphorrhea presentation. Product selection should follow wound assessment and local formulary guidance, not a brand name alone.

What Form of Compression May Be Used During Active Leakage?

During active volume change, an appropriately trained clinician may consider a multilayer inelastic or short-stretch bandaging system, a clinician-selected adjustable wrap, or another therapeutic compression method suited to the patient’s findings. Product instructions and local protocols should permit use over the selected dressing and wound presentation. These options may accommodate dressings and changing limb size more readily than a standard compression stocking.

Once drainage has stopped and volume is more stable, the plan may transition to a properly fitted garment, adjustable wrap, nighttime system, or another maintenance strategy. The sequence is individualized. Some patients cannot tolerate or self-manage the same system as others.

Compression selection should account for:

  • Arterial perfusion and cardiovascular stability
  • Edema cause, severity, and distribution
  • Wound and drainage characteristics
  • Limb shape and tissue fibrosis
  • Foot, toe, knee, thigh, or genital involvement
  • Neuropathy and protective sensation
  • Pain and skin fragility
  • Mobility, balance, and fall risk
  • Hand strength, cognition, vision, and caregiver availability
  • Ability to attend follow-up and obtain replacement supplies

This is one reason a loose elastic wrap applied without a pressure plan is not equivalent to therapeutic compression. Uneven tension, excessive local pressure, slippage, or a tourniquet effect can injure tissue and move edema into an untreated region.

What Should Clinicians Monitor After Compression is Applied?

Reassessment is essential, especially early in treatment or when drainage and limb volume are changing quickly. Monitor:

  • New or escalating pain
  • Numbness, tingling, weakness, or loss of sensation
  • Coolness, pallor, cyanosis, or other concerning color change
  • Pressure over bony prominences, tendons, fibular head, or wound edges
  • Rolling, slippage, gapping, or constriction
  • New swelling in the toes, foot, knee, thigh, or another area outside the system
  • Dressing saturation, strike-through, odor, and skin maceration
  • Changes in breathing, fatigue, weight, or systemic condition
  • The patient’s ability to walk, sleep, use footwear, and perform daily tasks

New neurovascular symptoms, severe pain, or systemic deterioration require immediate removal or modification according to clinical protocol and prompt reassessment. Patients and caregivers should receive clear instructions about which changes require an urgent call and which require emergency care.

What Does an Integrated Lymphorrhea Treatment Plan Include?

No single component addresses every part of a leaking, edematous limb. A coordinated plan commonly includes the following.

1. Gentle Cleansing and Protection of the Skin

The goal is to remove drainage and contaminants without adding friction or causing further barrier damage. Dry carefully, particularly between toes and within skin folds. Protect intact periwound skin with products appropriate to the patient’s skin, drainage, and sensitivities. Treat fungal infection, dermatitis, or other skin disease according to an established diagnosis and within professional scope.

Avoid sealing heavily draining tissue beneath a product that traps moisture without a plan for absorption and reassessment.

2. A Dressing Matched to the Fluid Burden

The dressing should be nontraumatic, sufficiently absorbent, and capable of retaining fluid under the chosen compression system. Initially, heavy drainage may require more frequent reassessment and changes. Frequency should decrease as clinically appropriate rather than following a fixed schedule regardless of strike-through or skin condition.

3. Therapeutic Compression, When Appropriate

Compression should address the involved region, protect vulnerable areas, and distribute pressure predictably. The clinician should know what system is being used, why it was chosen, how it is expected to behave at rest and during movement, and how response will be monitored.

4. Movement and Positioning that Fit the Patient’s Medical Status

Walking, ankle motion, and other prescribed exercise may improve muscle-pump activity when they are safe for the patient. Reduce unnecessary periods of dependency and use elevation or supportive positioning when tolerated and medically appropriate.

“Elevate and exercise” should not become generic advice detached from function. A patient with severe joint pain, a foot wound, dyspnea, weakness, or high fall risk may require a different movement plan, an assistive device, caregiver support, or medical evaluation.

5. Management of the Underlying Cause

This may involve vascular care, medical treatment of systemic disease, antibiotics for cellulitis, treatment of dermatitis or fungal infection, medication review, nutritional assessment, mobility rehabilitation, weight-management support, or oncology evaluation. The lymphedema clinician contributes edema-specific assessment and treatment while collaborating with the professionals responsible for the underlying condition.

6. Patient and Caregiver Education

Education should include:

  • How to recognize dressing strike-through and skin deterioration
  • Which infection, vascular, thromboembolic, and cardiopulmonary symptoms require escalation
  • How and when to perform prescribed skin care
  • Who should apply or adjust compression
  • What normal versus concerning sensations under compression may feel like
  • How to incorporate safe movement and positioning
  • What to do if supplies run out or the usual caregiver is unavailable
  • The follow-up plan after the leaking stops

The objective is not to hand the patient a complicated regimen. It is to make the plan understandable and achievable.

Where Does Manual Lymph Drainage Fit?

Manual lymph drainage may be one component of an individualized lymphedema program after acute infection, DVT, vascular compromise, and systemic instability have been considered. It may support management of edema in selected patients, particularly when integrated with compression, skin care, movement, and self-management.

MLD is not a substitute for controlling heavy drainage, protecting an open area, or selecting appropriate compression. Nor should every actively leaking limb automatically receive the same manual sequence. The condition driving the edema, altered lymphatic pathways, wound location, medical history, and patient tolerance all matter.

When cellulitis is suspected, prompt medical evaluation takes priority. Whether existing compression is continued, removed, modified, or resumed should be determined from the patient’s symptoms, perfusion, comfort, and the treating team’s protocol—not from a blanket rule applied to every infection.

Which Clinicians May Need to Collaborate?

Lymphorrhea often sits at the intersection of multiple services. Depending on the presentation, the care team may include:

Lymphedema therapist, wound-care nurse, and physician coordinating care with a patient.
  • A Certified Lymphedema Therapist
  • Wound or tissue-viability clinicians
  • Primary care or hospital medicine
  • Vascular medicine or vascular surgery
  • Cardiology, nephrology, or hepatology
  • Infectious disease
  • Dermatology
  • Oncology
  • Podiatry
  • Nutrition services
  • Home-health nursing
  • Physical or occupational therapy for mobility and self-management barriers

Clear ownership prevents a common failure: one clinician manages only the dressing, another assumes someone else is treating the edema, and no one defines the transition plan.

A useful shared plan identifies:

  • Who is evaluating the cause of the edema
  • Who is monitoring infection and systemic status
  • Who is responsible for the wound and skin plan
  • Who selects and adjusts compression
  • Who teaches the patient or caregiver
  • What outcomes will trigger escalation or a change in treatment
  • How the patient will obtain maintenance compression after closure

How Should Clinicians Measure Progress?

“The leg looks drier” is encouraging, but it is not enough for longitudinal decision-making. Document outcomes that reflect both the leak and the condition producing it:

  • Strike-through and periwound maceration
  • Limb circumference or volume at reproducible landmarks
  • Tissue texture and pitting
  • Skin condition and signs of inflammation or infection
  • Pain, heaviness, sleep, and comfort
  • Gait, mobility, and ability to wear footwear
  • Tolerance and integrity of the compression system
  • Patient or caregiver ability to carry out the plan
  • Episodes of cellulitis or unplanned medical care

Reassessment should be more frequent when the clinical picture is changing. A lack of expected improvement should prompt the clinician to reconsider diagnosis, perfusion, compression dosage or coverage, infection, systemic disease, adherence barriers, and the feasibility of the plan.

What Happens After the Leakage Stops?

Skin closure is a milestone, not the end of edema management. If the underlying pressure returns, leakage and skin breakdown may recur.

A maintenance plan may include:

  • Reassessment of limb size and shape
  • A properly fitted compression garment or adjustable system
  • Continued skin inspection and care
  • A sustainable movement program
  • Management of venous or systemic contributors
  • Footwear review
  • A replacement schedule for compression products
  • Training for the patient or caregiver
  • Follow-up when swelling, fit, skin, or health status changes

The patient should know what to do if swelling begins to increase before leakage returns. Earlier intervention may prevent another wet-leg episode and reduce the burden of repeated wound care.

Common Clinical Mistakes in Lymphorrhea Care

Treating the Drainage But Not the Edema

Repeatedly increasing dressing absorbency may be necessary in the short term, but it does not replace edema control when compression is safe.

Assuming Every Leaking Leg is Uncomplicated Lymphedema

New swelling or leakage can reflect venous disease, infection, systemic illness, medication effects, obstruction, or another diagnosis. The clinician should explain the change rather than relying on the label alone.

Applying Compression Without Adequate Screening

A familiar bandaging technique is not automatically appropriate for a patient with concerning arterial, cardiopulmonary, neurological, or infection findings.

Waiting for the Skin to be Completely Dry Before Addressing Edema

In many patients, correctly selected compression is part of how the leakage is brought under control. The answer is not indiscriminate compression, but prompt assessment and appropriate therapeutic compression when safe.

Ending Care When the Opening Closes

Without a maintenance strategy, the edema may return and place the repaired skin under stress again.

Designing a Plan the Patient Cannot Carry Out

Compression and wound care must fit the person’s mobility, dexterity, cognition, finances, support system, and daily life. Feasibility affects clinical effectiveness.

Why Lymphorrhea Management Requires Specialized Clinical Reasoning

Managing lymphorrhea is not simply a matter of choosing the most absorbent dressing or applying the strongest compression available. Clinicians must connect lymphatic physiology, venous disease, wound care, arterial perfusion, skin integrity, movement, medical stability, and patient education.

That requires knowing both how to intervene and when to pause, modify, collaborate, or refer.
