Swelling is common, but not all swelling is lymphedema—and not every swollen limb should immediately be managed with compression, manual lymph drainage, or other components of Complete Decongestive Therapy (CDT).
For clinicians working with patients who have known or suspected lymphedema, one of the most important skills is recognizing when the presentation does not fit the expected pattern. Sudden swelling, unusual pain, skin changes, systemic symptoms, vascular compromise, or changes in cardiopulmonary status may indicate a condition that requires further medical evaluation before routine lymphedema care proceeds.

Although lymphatic dysfunction may be responsible for a patient’s swelling, edema can also occur with venous disease, cardiac disease, renal dysfunction, infection, vascular obstruction, medication use, cancer, trauma, and other conditions.
Patients may also have more than one contributing factor.
For example, a patient may have established lower-extremity lymphedema but later develop cellulitis. Another may have chronic venous insufficiency combined with lymphatic dysfunction. A cancer survivor with stable secondary lymphedema may suddenly develop new swelling that deserves further investigation.
The goal of screening is therefore not simply to determine whether lymphedema exists. It is to answer a more immediate question:
Is it appropriate and safe to begin the planned intervention today?
A clinician should consider:

- How quickly did the swelling develop?
- Is the swelling unilateral or bilateral?
- Is this a new problem or a change from the patient’s baseline?
- Is there pain, redness, warmth, or tenderness?

Red Flag #1: Sudden Unilateral Swelling

New, rapid, unexplained unilateral swelling deserves attention.
One possible cause is deep vein thrombosis (DVT), particularly when swelling occurs alongside symptoms such as:

- New calf or leg pain
- Tenderness
- Warmth
- Skin discoloration
- A feeling of heaviness or tightness
- A significant difference between limbs
The absence of dramatic symptoms does not completely exclude a thrombotic event, which is why clinicians should avoid trying to diagnose or rule out DVT based solely on appearance.

- Rapidly spreading redness
- Fever or chills
- Increasing pain
- Malaise or flu-like symptoms
- Drainage or an obvious infected wound
- Red streaking
- Significant deterioration from the patient’s usual condition
More severe or rapidly progressing skin and soft-tissue infections can become medical emergencies, making early recognition particularly important. IDSA guidelines emphasize that skin and soft-tissue infections range from relatively limited infections to potentially life-threatening disease.

A red, hot, painful limb should never automatically be labeled a “lymphedema flare.”

Red Flag #3: Signs of Significant Arterial Insufficiency
Compression is one of the most important tools in lymphedema management—but compression must be selected with the patient’s vascular status in mind.
Lower-extremity peripheral artery disease (PAD) reduces arterial blood flow to the legs and feet. In patients with concerning symptoms or examination findings, vascular assessment may be needed before significant compression is initiated.

- A cool foot
- Pallor or abnormal discoloration
- Weak or absent pedal pulses

The 2024 multisociety guideline for lower-extremity peripheral artery disease recommends resting ankle-brachial index testing when history or physical examination findings suggest PAD.
The important lesson is not that every patient with arterial disease can never receive compression.
Rather, compression intensity must be appropriate for the individual’s arterial circulation and overall clinical status.
Some patients with mixed arterial and venous or lymphatic disease may still receive carefully selected compression under appropriate medical guidance. Severe ischemia, however, requires a very different approach from uncomplicated lymphedema.

Before applying compression to a lower extremity, understand the circulation beneath it.
Red Flag #4: New or Worsening Shortness of Breath
A patient arrives for lymphedema therapy and reports that their legs have become more swollen during the past week.
That may initially sound like a lymphedema problem.

Then they mention:
“I’ve also been getting short of breath walking from the bedroom to the kitchen.”
That changes the clinical picture.
New or worsening bilateral lower-extremity edema accompanied by cardiopulmonary symptoms may suggest systemic fluid overload or another medical problem that needs evaluation.
Concerning associated symptoms can include:
- New or worsening shortness of breath
- Difficulty breathing while lying flat
- Rapid unexplained weight gain
- Markedly increasing bilateral edema
- New chest discomfort
- Unusual fatigue
- Reduced exercise tolerance
- Changes in known heart failure symptoms
Heart failure itself should not be reduced to a simplistic “never compress” rule. Compression considerations depend on cardiovascular stability, severity, and medical management. Published international consensus recommendations on compression emphasize differentiating stable cardiovascular disease from severe or decompensated heart failure rather than treating all cardiac disease identically.
Clinical takeaway
The key question is not simply, “Does this patient have a cardiac history?”
Ask: “Is this patient medically stable today?”
Red Flag #5: Sudden, Unexplained Changes in a Previously Stable Lymphedema Patient
Long-term patients provide clinicians with something extremely valuable: a baseline.
You may know that a patient’s right arm normally measures a certain size, that their tissue feels mildly fibrotic, or that swelling typically improves overnight.
A sudden departure from that pattern deserves attention.
Examples include:

- Rapidly increasing limb volume
- Swelling appearing in a previously unaffected region
- New pain
- Unexpected firmness
- New neurological symptoms
- An unexplained mass
- New enlarged lymph nodes
- Unusual skin changes
- Loss of response to a previously effective management program
In patients with a history of cancer, unexplained changes should be assessed carefully rather than automatically attributed to pre-existing lymphedema.
The International Society of Lymphology notes that medical evaluation is essential because other conditions—including malignant obstruction—can contribute to lymphatic swelling.
This does not mean that people with active or previous cancer should automatically be excluded from lymphedema therapy. Oncology rehabilitation is an important area of lymphedema practice.

Red Flag #7: Skin Changes Suggesting Ischemia, Necrosis, or Serious Infection

Dryness, hyperkeratosis, papillomatosis, fibrosis, lymphorrhea, and chronic wounds may all be encountered in advanced lymphatic disease.

- Significant color or temperature changes
These findings may require wound, vascular, or medical evaluation before routine lymphedema interventions proceed.

Do not ask only, “Is there a wound?”
Ask: “Why is this wound here, and what does the surrounding tissue tell me?”
Red Flag #8: Swelling That Does Not Fit a Typical Lymphedema Pattern
Not every enlarged limb is lymphedema.
Differential assessment matters.

Clinicians should think more broadly when the patient’s presentation includes:

- Generalized edema

Red Flag #9: A Significant Change in Medical Status
Sometimes the most important red flag appears in the history rather than the limb.
Before initiating or resuming lymphedema care, clinicians should ask about recent changes such as:

- Hospitalization
- New cardiovascular diagnosis
- Recent surgery
- New blood clot
- Acute infection
- Significant medication changes
- New cancer diagnosis or recurrence
- New renal or hepatic disease
- Major change in mobility
- Recent unexplained falls or neurological symptoms
A compression prescription or home program that was appropriate six months ago may require reassessment after a major change in health.
This is one reason routine history-taking remains important even with established patients.
Red Flag #10: Something Simply Does Not Fit
Clinical intuition should not replace evidence—but experienced clinicians learn to recognize when the pieces of a presentation do not line up.
Perhaps the swelling developed too rapidly.
Perhaps the pain is unusual.
Perhaps the skin temperature is different.
Perhaps the patient seems much more fatigued than usual.
Perhaps the medical history does not explain what you are seeing.
A therapist does not need to diagnose every possible medical condition to recognize that additional evaluation is warranted.
One of the safest clinical decisions can be:
“Before we proceed, I would like this change evaluated.”
Referral is not a failure to provide care.
It is part of responsible care.
Red Flag Does Not Always Mean “Never Treat”
This distinction deserves emphasis.
Clinicians sometimes learn contraindications as rigid lists:
DVT = no therapy.
Heart disease = no compression.
Cancer = no manual lymph drainage.
Wound = no compression.
Real clinical practice is more nuanced.
A suspected acute DVT is different from a medically managed history of DVT.
Decompensated heart failure is different from stable, medically managed cardiac disease.
A new unexplained mass is different from a patient receiving oncology rehabilitation.
Severe arterial insufficiency is different from mild or moderate vascular disease managed with an appropriately modified compression plan.
And an infected wound is very different from a stable wound being managed within an interdisciplinary wound-care program.

- Acute limb ischemia
- New severe cardiopulmonary symptoms
- Rapidly progressive unexplained swelling
- Severe unexplained pain
- Signs of tissue necrosis
- Other signs of an acute medical emergency
2. Coordinate Care or Obtain Additional Assessment
Examples may include:
- Known arterial disease
- Significant cardiac disease
- New unexplained edema
- Complex wounds
- Recent major medical changes
- Possible systemic causes of edema
- New findings in a patient with cancer history
3. Proceed With Appropriate Modifications and Monitoring
Many chronic comorbidities do not automatically prevent lymphedema care.
Once medical stability and relevant precautions are established, the clinician may modify:
- Compression pressure
- Garment selection
- Bandaging technique
- Exercise intensity
- Treatment positioning
- Session length
- Frequency of skin checks
- Patient education
- Follow-up schedule

There is equal value in recognizing when today’s swelling may be telling you something else.
Before beginning lymphedema care, clinicians should evaluate the entire patient—not simply the enlarged limb.
A sudden change in swelling, suspected infection, vascular compromise, severe pain, cardiopulmonary symptoms, unexplained tissue changes, or a significant departure from the patient’s normal presentation should prompt clinical reconsideration.

Sometimes that means modifying the plan.
Sometimes it means collaborating with another provider.
And sometimes the safest decision is to postpone routine lymphedema intervention until the patient has received appropriate medical evaluation.
