CDT GROUP STUDENT AGREEMENT
Welcome to the Norton School of Lymphatic Therapy CDT Certification Course online registration process. CDT Certification Course Group Agreement
The following agreement is made between the students registering for the course and the Norton School of Lymphatic Therapy.
Confirmation
After acceptance into the course, students will receive a confirmation letter indicating the exact starting and ending times as well as pertinent information about the location of the course.
Class Times
A total of 135 hours of instruction is provided in the CDT Certification Course comprised of 90 hours of classroom instruction and 45 hours of home study. Class hours are 8:00am–7:00pm.
Home Study Program
The Home Study Program is comprised of the following components: (1) multimedia "Nine-Part Theoretical Home Study" online course; multimedia "Lymphedema Bandaging: Video Practical Instruction" online course; multimedia "Manual Lymph Drainage: Video Practical Instruction" online course; (2) various online theoretical reference components; (3) a Course Manual and Online Reference Guide; (4) online access to video content covering various lymphedema topics; (5) "Lymphedema Management" textbook co-authored by Steve Norton. 45 hours of home study are expected of students. The Home Study Program will provide an opportunity for students to develop an extensive knowledge of the theoretical components of lymphedema therapy.
Completion of Home Study Program
Students agree to complete the Home Study Program in its entirety prior to the commencement of the live class as outlined in the document "Home Study Program" provided to students at the time of registration. Students pledge to be the only individuals accessing and completing the Norton School of Lymphatic Therapy Home Study Program. The Home Study Program materials, as well as any other work required for completion of the Home Study Program, are strictly prohibited from being shared with other individuals or organizations.
Tuition
The group tuition for the CDT Certification Course is as follows:
2 students $3,000 per student
3 students $2,900 per student
4 students $2,800 per student
5 students $2,700 per student
A deposit is required at the time of registration and will hold spots in the course until the remaining balances are received no later than two (2) weeks prior to the first day of class. The group deposit is as follows:
2 students $700 per student
3 students $600 per student
4 students $500 per student
5 students $400 per student
Tuition and deposit amounts are payable in U.S. currency only.
Refund Policy
$250 of the deposit is non-refundable. Upon receipt of the Home Study Program materials, $650 will become non-refundable. $900 will become non-refundable two (2) weeks prior to the first day of live class. The full tuition will become non-refundable after the live class has begun. Refunds are processed in the order they are requested and are typically issued 4-6 weeks from the date of request.
Attendance
100% attendance is expected of students. Any student missing more than five (5) hours of the class may not receive a certificate of completion.
Certificate of Completion
In order to receive a certification of completion, students must pass the written and practical exams as required by the instructor.
Purpose of the Training
Students agree that the purpose of the training is to become proficient in the techniques of Manual Lymph Drainage and Complete Decongestive Therapy. Although students are free to distribute information concerning lymphedema upon completion of the course, students shall not under any circumstances train or certify any other person in MLD or CDT unless granted instructor status by the Norton School of Lymphatic Therapy.
Transfer
Registered students are eligible to transfer to another CDT Certification Course once within 12 months of the original registration date without incurring a transfer fee. Subsequent transfers will incur a $200 transfer fee. The fee will accrue against a student's credit balance. A student may only transfer if the credit balance is sufficient to cover the transfer fee. Once a student commences taking a CDT Certification Course, all accrued transfer fees will be waived. In the event a student does not transfer to another course and does not commence a CDT Certification Course within a continuous 12-month period, the accrued transfer fees will be deducted from the student's credit balance and the remaining credit balance will be refunded in accordance with the refund policy.
Withdrawal
Withdrawal from the course may result in a loss of tuition payments. If a student cannot complete the course in its entirety due to personal emergency or medical reasons, a 12-month grace period will be granted from the original date of the live course. During this grace period the student may contact the Norton School of Lymphatic Therapy to arrange a return date for a scheduled course to reserve a seat. At the Norton School of Lymphatic Therapy's discretion, the student's participation in the course may be abbreviated to include only days missed rather than the entire course. If coursework is not satisfactorily completed during the grace period all tuition payments will be forfeited.
Dismissal
Any student may be dismissed from the course for insufficient progress, non-payment of tuition, non-attendance, or unprofessional/unethical conduct. Reinstatement following dismissal is at the discretion of the instructor and/or the Norton School of Lymphatic Therapy.
Cancellation of Classes
If the course is cancelled because of insufficient registration, tuition payments can be refunded or transferred to another class as elected by registrants. If the course is cancelled for any other reason, a new course will be scheduled and tuition payments will be transferred. In the event the instructor becomes ill, the class will be rescheduled.
Documentation Requirement
After completing the online group registration process, please email, fax, or mail us: (1) the registration information and a signed student agreement for each additional registrant beyond the primary registrant; (2) a copy of each registrant's professional license/diploma. We require these documents for every registrant to complete the group registration.
As the primary registrant for this group registration, I understand and agree to the terms and conditions of this agreement. I further agree that checking the "I Agree to the Terms and Conditions" box, entering my full name in the "Full Name" box, and entering a valid e-mail address in the "E-Mail Address" box collectively constitute an electronic signature thereby rendering this Agreement valid and legally binding.
