For many survivors, the next phase of healing includes restoring movement, rebuilding strength, managing fatigue, reducing pain, adapting to body changes, and addressing long-term side effects that may appear months or even years after cancer treatment.

One of the most important, and sometimes underestimated, needs in oncology rehabilitation is lymphedema care.

Lymphedema can occur when cancer itself, surgery, lymph node removal, radiation therapy, infection, or scarring disrupts the normal flow of lymphatic fluid. When lymphatic pathways are damaged or overloaded, fluid may build up in the tissues, leading to swelling, heaviness, tightness, discomfort, skin changes, reduced mobility, and increased risk of infection. The National Cancer Institute explains that lymphedema may develop when cancer or cancer treatment disrupts lymph flow or damages lymph nodes and vessels.

For cancer rehabilitation teams, this creates a clear need: patients require clinicians who understand oncology, rehabilitation, and lymphatic care. That is where Certified Lymphedema Therapists, often referred to as CLTs, become essential members of the cancer care team.

A CLT is trained to evaluate and manage lymphedema using specialized skills such as manual lymph drainage, compression bandaging, skin care education, exercise instruction, garment recommendations, risk-reduction strategies, and long-term self-management planning. For oncology rehab programs, having CLTs on the team can improve continuity of care, help patients recognize symptoms earlier, and support survivors through a complex and often emotional recovery process.

Why Cancer Survivors Are at Risk for Lymphedema

The lymphatic system plays a vital role in fluid balance, immune function, and tissue health. It helps move lymph fluid through a network of vessels and nodes, filtering waste and supporting the body’s defenses. When cancer treatment affects this system, lymph fluid may not drain as efficiently as it once did.

Cancer-related lymphedema is often associated with breast cancer treatment, but it can also occur after treatment for melanoma, gynecologic cancers, prostate cancer, head and neck cancers, sarcoma, and other cancers involving lymph node removal, radiation, or tumor-related lymphatic obstruction.

Common risk factors may include:

Lymph node removal or biopsy

Radiation therapy to lymph node regions

Surgery that disrupts lymphatic vessels

Tumor involvement or obstruction of lymphatic pathways

Postoperative scarring or fibrosis

Infection or cellulitis

Reduced mobility after surgery or treatment

Higher body weight or significant weight changes

History of trauma or injury to the affected region

The American Cancer Society notes that radiation therapy can damage or scar lymph nodes, making it harder for them to work properly and increasing the chance of lymph fluid buildup.

This is one reason oncology rehab teams need to think beyond immediate postoperative recovery. A patient may appear to be recovering well from cancer treatment, but still carry a lifelong risk of lymphedema. Some patients develop swelling shortly after treatment, while others develop symptoms much later.

Why Early Recognition Matters

In oncology rehab, timing can make a major difference. Lymphedema is often easier to manage when it is identified early. Subtle symptoms may appear before dramatic swelling occurs, and patients may not know that those symptoms are important.

Early signs can include:

A feeling of heaviness in the arm, leg, breast, chest wall, neck, face, or trunk

Tightness in the skin or clothing

Jewelry, sleeves, bras, waistbands, or shoes fitting differently

Aching, fullness, or pressure

Reduced flexibility or range of motion

Mild swelling that comes and goes

Skin texture changes

Increased sensitivity or discomfort in the affected area

Without proper education, patients may dismiss these symptoms as normal soreness, scar tightness, weight gain, or general post-treatment discomfort. A CLT can help distinguish between expected healing and signs that require further attention.

Early intervention may include education, exercise, compression recommendations when appropriate, manual lymph drainage, skin care instruction, and monitoring. In some cases, the goal is not only to reduce swelling but to prevent progression, preserve function, and help the patient feel confident managing risk.

Oncology Rehab Requires More Than General Rehabilitation Skills

Physical therapists, occupational therapists, athletic trainers, nurses, and other rehabilitation professionals may already have strong foundations in anatomy, movement, tissue healing, patient education, and functional recovery. However, lymphedema care requires additional specialized training.

Cancer survivors often present with layered clinical considerations. A patient may have shoulder range-of-motion limitations after breast surgery, fatigue from chemotherapy, neuropathy, radiation fibrosis, scar sensitivity, fear of movement, and early-stage lymphedema risk all at once. Another patient may be managing head and neck swelling after radiation while also struggling with swallowing, speech, posture, and skin changes.

These cases require a clinician who can think across systems.

A CLT is trained to understand:

Lymphatic anatomy and physiology

Lymphedema staging and presentation

Manual lymph drainage principles

Compression bandaging techniques

Garment selection and patient education

Skin care and infection risk reduction

Therapeutic exercise for lymphatic support

Contraindications and precautions

Cancer-related modifications

Long-term self-management planning

This is especially important because oncology rehab patients may have medical complexity. Clinicians must know when to proceed, when to modify care, when to pause, and when to refer back to the oncology team. A focused review on cancer rehabilitation safety considerations highlights the importance of understanding cancer-related medical status, treatment side effects, and safety concerns when providing rehabilitation services.

The Role of Complete Decongestive Therapy in Cancer-Related Lymphedema

Complete Decongestive Therapy, or CDT, is widely recognized as a central approach to lymphedema management. CDT typically includes manual lymph drainage, compression therapy, exercise, skin care, and patient education. A 2023 review describes complex physical decongestive therapy as the standard therapy for lymphedema and identifies manual lymph drainage, compression therapy, exercise, and skin care as major components.

For oncology rehab patients, CDT is not a one-size-fits-all protocol. It must be adapted to the patient’s cancer history, surgical procedures, radiation fields, tissue quality, pain levels, endurance, skin integrity, and personal goals.

A breast cancer survivor with mild upper extremity swelling may need education, measurement, exercise progression, compression guidance, and monitoring. A patient with more advanced swelling may need intensive CDT with multilayer bandaging and a transition into daytime and nighttime compression. A head and neck cancer survivor may need specialized techniques that address facial, submental, or cervical swelling while considering skin sensitivity, radiation changes, and swallowing or breathing concerns.

The clinical reasoning behind CDT is what makes CLT training so important. The therapist is not simply following a checklist. The therapist is assessing, adapting, teaching, and coordinating care.

Where CLTs Fit on the Cancer Care Team

A strong oncology rehabilitation program is multidisciplinary. Patients may interact with surgeons, medical oncologists, radiation oncologists, oncology nurses, physical therapists, occupational therapists, speech-language pathologists, dietitians, social workers, mental health professionals, prosthetists, orthotists, and compression garment fitters.

CLTs help bridge several of these areas.

A Certified Lymphedema Therapist may support the team by:

Screening patients for lymphedema risk

Educating patients before or after surgery

Monitoring limb or regional volume changes

Addressing early swelling, heaviness, or tissue changes

Supporting shoulder, trunk, neck, or limb mobility

Teaching safe movement and exercise

Recommending compression strategies

Helping patients transition from active therapy to self-management

Communicating with oncology providers about changes or concerns

Referring patients for medical evaluation when red flags appear

This role is not limited to breast cancer care. Oncology rehabilitation professionals may encounter lymphedema related to pelvic cancer treatment, melanoma surgery, head and neck radiation, prostate cancer treatment, sarcoma surgery, or other cancer care pathways.

The more a cancer program understands lymphedema, the better prepared it is to serve survivors across diagnoses.

Breast Cancer Rehab and Lymphedema: A Common Example

Breast cancer rehabilitation is one of the most recognized areas where CLTs can make a significant impact. Patients may experience arm swelling, breast swelling, chest wall swelling, axillary web syndrome, scar restrictions, shoulder stiffness, pain, weakness, and fear about using the affected arm.

Clinical practice resources from APTA address breast cancer-related lymphedema and the physical therapist’s role in lymphedema care.

For these patients, a CLT can help with more than swelling. The therapist can support function, body confidence, comfort, and education. Patients often want to know whether they can return to exercise, lift groceries, travel, sleep on the affected side, garden, swim, or resume work. A trained therapist can help them move forward safely while understanding their risk profile.

This matters because patients are often trying to rebuild trust in their bodies after a difficult medical experience. Clear education from a skilled clinician can reduce fear and improve participation in daily life.

Head and Neck Cancer Survivors Also Need Lymphedema Support

Head and neck lymphedema can be especially challenging because it may affect visible areas of the face and neck, as well as comfort, posture, swallowing, speech, and quality of life. Radiation and lymph node treatment in the neck can contribute to swelling, fibrosis, and tissue restriction.

These patients may not always recognize swelling as lymphedema. They may describe tightness under the chin, facial fullness, difficulty with collars, pressure in the neck, or changes in jaw and neck mobility.

CLTs who work with head and neck cancer survivors need specialized knowledge and careful clinical reasoning. The anatomy is complex, the tissues may be sensitive, and treatment plans often require coordination with speech-language pathology, oncology, dentistry, nutrition, and surgical teams.

For oncology programs that treat head and neck cancer, CLT training can be a meaningful addition to patient care.

Pelvic, Genital, and Lower Extremity Lymphedema After Cancer Treatment

Lymphedema after gynecologic, prostate, colorectal, melanoma, or pelvic cancer treatment can affect the legs, pelvis, abdomen, or genital region. These presentations can be physically and emotionally difficult for patients to discuss.

Patients may report leg heaviness, swelling that worsens through the day, difficulty with clothing, pelvic pressure, genital swelling, skin irritation, or trouble walking comfortably. Because these symptoms can feel private or embarrassing, patients may delay seeking help.

A CLT can create a safer clinical environment by asking thoughtful questions, normalizing discussion, and explaining how cancer treatment can affect lymphatic drainage. This helps patients feel less isolated and more willing to participate in care.

Lower body and pelvic lymphedema can also require careful garment planning, compression strategies, skin care routines, and exercise modifications. In oncology rehab, these patients benefit from clinicians who understand both the physical and emotional dimensions of survivorship.

CLTs Help Patients Understand Compression

Compression can be one of the most confusing parts of lymphedema care for patients. They may be unsure whether they need a sleeve, glove, gauntlet, stocking, wrap, bra, vest, head and neck garment, chip pad, foam insert, or nighttime garment. They may also struggle with fit, comfort, donning, skin sensitivity, and long-term adherence.

A CLT helps patients understand why compression is recommended, how it works, and how it fits into daily life.

This education is especially important for cancer survivors because compression may feel like another unwanted reminder of cancer treatment. Patients may resist it at first because it looks medical, feels uncomfortable, or seems difficult to manage. A knowledgeable therapist can explain options, troubleshoot barriers, and collaborate with garment fitters or suppliers to help patients find a workable solution.

The best compression plan is not just clinically appropriate. It must also be realistic for the person wearing it.

CLTs Support Long-Term Survivorship

Cancer survivorship is not a short period of recovery. Many patients live for years or decades after cancer treatment, and long-term side effects may affect daily function, work, exercise, sleep, self-image, and emotional health.

Lymphedema management often becomes part of that long-term survivorship plan.

CLTs help patients build routines around:

Skin inspection

Infection risk reduction

Compression use

Exercise and mobility

Weight and wellness conversations

Travel preparation

Symptom monitoring

Flare-up planning

Follow-up care

When to contact a medical provider

The goal is not to make patients dependent on therapy forever. A systematic review of cancer rehabilitation guidelines found that rehabilitation services are recommended across many cancer types and for many treatment-related consequences.

This growth creates an opportunity and a responsibility. Cancer survivors need access to clinicians who understand more than general strengthening and range of motion. They need providers who can recognize lymphatic risk, screen for swelling, educate patients clearly, and deliver skilled lymphedema care when needed.

Certified Lymphedema Therapists help meet that need.

Bringing CLT Skills Into Cancer Care

Lymphedema therapy is not a separate, isolated specialty outside of oncology rehabilitation. It is deeply connected to cancer survivorship, functional recovery, patient education, and quality of life.

Cancer care teams need CLTs because lymphedema can affect how patients move, dress, work, exercise, sleep, socialize, and feel in their own bodies. It can influence infection risk, skin health, pain, mobility, and emotional well-being. It can appear early or late. It can be mild or complex. It can affect the arm, leg, breast, chest wall, abdomen, genitals, face, or neck.

Most importantly, it requires knowledgeable care.

When oncology rehab teams include Certified Lymphedema Therapists, they are better prepared to support survivors through one of the most common and challenging long-term effects of cancer treatment.

For clinicians, CDT certification offers a way to deepen expertise, expand career opportunities, and provide a service that many cancer survivors urgently need.

Ready to Strengthen Your Oncology Rehab Skill Set?

Norton School of Lymphatic Therapy provides comprehensive Complete Decongestive Therapy training for healthcare professionals who want to become Certified Lymphedema Therapists.
